Children must be 8 years of age or older
Complete one Chevrolet Junior Pride Crew application packet per child, including an application, waiver, and payment form
To request an application packet by mail, please email dmielke@chisox.com
Mail the completed packet to:

All forms must be received for final consideration

Participation Fee: $250.00 per child

Chevrolet Junior Pride Crew members receive:

Two game tickets and one parking pass per experience
- Up to four additional tickets may be purchased at regular price (subject to availability)
- Additional tickets must be purchased three weeks in advance of game date by calling 312-674-5368 or email dmielke@chisox.com
- Please note: This offer does not include $1 day-of-game purchase of Kids' Day tickets

June - 5th, 6th, 7th, 10th
July -3rd, 4th, 5th, 8th, 24th, 25th
August - 7th, 8th, 26th
September - 4th, 5th, 11th, 12th, 13th

Space is limited.
Applications will be reviewed on a first-come, first-served basis. Payment will only be processed upon confirmation, if space permits and application is accepted.
