GRAND SLAM GRANTS DISBURSEMENT GUIDELINES

Grand Slam Grants

In order to be considered for Grand Slam Grants funding, all organizations submitting an application MUST have registered charitable status OR non-profit status, MUST target children and youth in need, and MUST meet at least one of the following criteria:

  1. 1. Physical Activity – includes programs that promote healthy physical activity for children and youth at all ages and skill levels
  2. 2. Education – includes mentoring/tutoring programs, literacy efforts, and programs that encourage academic achievement
  3. 3. Life-Skill Development – includes programs that focus on leadership, teamwork and character development, youth health and welfare, substance abuse education, healthy living and safety initiatives, and self-esteem development

 

Please be advised:
  • 2011 application is now closed
  • Grand Slam Grants will be awarded to a maximum of $50,000 per year, however, successful grant applications may receive only partial funding.
  • Organizations that have received funding from Jays Care Foundation in previous funding cycles must resubmit a proposal in order to be eligible for future funding.
  • All successful applicants will be required to sign a Letter of Agreement prior to receiving funds from Jays Care Foundation.
  • Successful applicants must file a Post Grant Report to Jays Care Foundation within ninety (90) days of the completion of the project. The final report shall evaluate the success of the project and include a budget that itemizes how the funds were utilized.
  • Future funding requests from organizations with outstanding Post Grant Reports will not be considered.

GRAND SLAM GRANTS APPLICATION PROCESS

To ensure that submissions are responded to in a timely and efficient manner, all organizations MUST complete a Jays Care Foundation Grand Slam Grants Application IN FULL in order to be considered for funding. Incomplete applications may not be considered.

We strongly suggest that you download the Grand Slam Grants Application Form as a Microsoft Word document, enter the information into the document, save and print.

Please be advised:
  • All correspondence must be typed
  • All pages must be numbered
  • All copies of the application must be collated and stapled or clipped individually
  • Applications may not be submitted by e-mail or fax
  • Postmark dates are not considered, any applications after the deadline may not be considered during the corresponding review cycle
  • Applicants are strongly encouraged to ensure a copy of the application is kept on file
  • The review process may take 5-6 months, please plan accordingly
